Job Description
Whitney Nursing and Rehabilitation is hiring a full-time Licensed Social Worker to handle discharge planning and care coordination. You will work directly with families and interdisciplinary staff to ensure care plans match resident medical goals. The day-to-day focus centers on completing social assessments and organizing care transitions for incoming and outgoing residents. Responsibilities Complete social service assessments and progress notes for facility residents. Coordinate discharge plans with home health agencies and medical equipment vendors. Assist residents and family members with Medicaid applications and paperwork. Lead care plan conferences alongside nursing leadership and therapy staff. Address daily resident non-clinical needs and coordinate with floor staff on care updates.
