The Licensed Practical Nurse (LPN) works in conjunction with a medical provider to provide professional, team based medical care and nursing support in a clinical setting. Under clinical supervision of the Lead Nurse, Unit Lead, Associate Director of Nursing (ADON), or provider and the Chief Nursing Officer (CNO), the nursing staff plays a critical role in the delivery of quality, patient-centered medical care within the context of a federally (HRSA) qualified community health center. (FQHC).

Essential Duties and Responsibilities:

Patient Relations:

1. Communicates with patients in a courteous and professional manner in face-to-face encounters and via the telephone. 2. Educates the patient and family members on the patient's diagnoses' and health care self management within scope of knowledge and authority.

Staff Relations/Team Building:

1. Participates with the provider in the implementation of the plan of care and multidisciplinary patient education. 2. Promotes the Team-Based Care Model by participating in daily huddles and monthly care team meetings to communicate and collaborate on patient issues and needs.

Clinical Duties:

1. Conducts patient screening for hearing, vision, vital signs to include blood pressure, height, weight, and other routine screening procedures as needed. 2. Assists the providers with in-office tests and procedures as ordered. 3. Administers medications including intramuscular (IM), subcutaneous, and intradermal injections as ordered by the provider. 4. Documents clinical activities and patient interactions in the medical record. 5. Collects specimens from patients, performs in-house laboratory testing, and processes specimens and requisitions for transport to the outside testing facility. 6. Stocks exam rooms and special treatment rooms with necessary supplies and indicates necessity for ordering supplies on the appropriate supply order form or on the designated board or list. 7. Demonstrates enthusiasm and commitment toward continuous learning and developmental opportunities. 8. Returns patient phone calls promptly. 9. Demonstrates flexibility, open mindedness, and versatility in adjusting to changing environments and requirements as necessary. 10. Functions as a triage nurse. 11. Interacts with other agencies/medical entities as needed to obtain patient medical information, to ensure continuity of care, and to ensure patients receive necessary durable medical equipment (DME), prescriptions, and other supplies. 12. Participates in education of nursing students, medical assistant students, and new hires as assigned. 13. Other clinical duties and responsibilities as assigned by the Lead Nurse, Unit Lead, ADON, or CNO. 14. Physical attendance is an essential element of the job and necessary to perform the essential functions of the LPN position.

Administrative Duties:

1. Attends and participates in JHC and other meetings as required. 2. Will supervise nursing and support personnel as assigned.

Performance Improvement Activities:

1. Attends all nursing department meetings and participates in the JHC Quality Improvement (QI) program.

Safety/Infection Control Activities:

1. Attends all mandatory OSHA infection control trainings and follows general OSHA guidelines for infection control, including "Standard Precautions". 2. Abides by all JHC infection control policies, including acute response measures in the setting of a public health concern or disease outbreak.

Competencies:

1. Performs skills evaluation a minimum of every two years utilizing the RN/LPN/Medical Assistant Competency Skills Form.

Other Functions:

1. Staff will abide by the Code of Conduct as documented in the Corporate Compliance Manual. 2. Must demonstrate a personal and professional commitment to Johnson Health Center and its mission. 3. Treats all patients and staff with dignity and respect being mindful of the cultural differences of the diverse population we serve. 4. Management may modify, add, or remove any job functions as necessary, or as changing organizational needs require.

Qualifications:

1. Must be licensed with the Virginia State Board of Nursing with a current LPN license. 2. Must have graduated from an accredited nursing program. 3. Minimum 1-3 years' experience as an LPN preferred. 4. Ability to work a flexible schedule

Physical Demand and Working Environment:

Fast paced office setting with travel to other offices often. Lifting and/or exerting force up to 25 pounds occasionally, with frequent movement of objects. Work requires speaking, sitting, bending, walking, standing, hearing, stooping, kneeling, and repetitive motion with certain activities. 8 or more hours of computer usage. OSHA high-risk position.
