Licensed Practical Nurse - Home Health Nurse
Job Description
Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our home health care team. The ideal candidate will possess a strong foundation in patient care, with experience working in diverse clinical settings. As a Home Health Nurse, you will deliver personalized nursing services directly to patients in their homes, promoting health, safety, and independence. This role offers an opportunity to make a meaningful difference in the lives of individuals requiring ongoing medical support and assistance. Responsibilities Provide comprehensive nursing care to patients in their homes, including assessments, vital signs monitoring, and medication administration. Develop and implement individualized care plans based on patient needs and physician directives. Administer treatments such as IV infusions, catheter care, and wound management while adhering to sterile techniques and safety protocols. Conduct patient evaluations, including physical examinations and diagnostic evaluations, ensuring accurate documentation within electronic health records (EHR/EHR systems). Educate patients and caregivers on disease management, medication usage, and health maintenance strategies. Coordinate with interdisciplinary teams to ensure seamless delivery of care, including case management and discharge planning. Maintain compliance with HIPAA regulations and confidentiality policies while managing sensitive medical records. Utilize EMR systems such as Epic or Athenahealth for documentation, scheduling, and communication purposes. Experience Proven experience as an LPN with a background in home health or community care settings. Critical care experience or hospital experience in emergency medicine, ICU, or trauma centers is highly desirable. Familiarity with medical office procedures, medical terminology, ICD coding (ICD-9/ICD-10), and documentation standards. Experience working with individuals with developmental disabilities or dementia-related conditions such as Alzheimer's disease is preferred. Knowledge of infection control practices, sterile processing, and aseptic techniques essential for safe patient care. Prior experience with IV insertion, phlebotomy, medication administration—including injections—and tube feeding is advantageous. Ability to assess patient physiology and perform physical examinations accurately within a home environment. Strong communication skills for caregiver communication, patient education, and triage responsibilities across diverse populations including geriatrics and pediatrics.
