About the
Role:
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our interdisciplinary Assertive Community Treatment (ACT) team. In this role, you will deliver hands-on medication administration and clinical health support directly to adults with severe and persistent mental illness in their natural environments, helping them achieve stability and community integration.Key Responsibilities:
Administer and monitor psychotropic medications, including long-acting injectables (LAIs), in community settings. Maintain accurate, up-to-date Medication Administration Records (MAR) for all doses delivered. Provide health literacy counseling and medication education to clients and family support systems. Collaborate closely with the Psychiatric Care Provider regarding side-effect monitoring and medication adjustments. Participate in daily team meetings, caseload reviews, and community crisis response.Qualifications:
Active LPN license issued by the Virginia Board of Nursing or multistate privilege. Minimum 1 year of clinical experience involving medication management preferred. Strong clinical documentation, crisis communication, and interpersonal skills.Pay:
