Job Description
1st Americare is seeking an LPN or RN to work with our client in the Woodbridge area. Candidate with Trach, Vent, & Gtube on a pediatric experience is required. The schedule for this case will be: Saturdays 11pm-7am
Duties and Responsibilities:
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and adjust care plans accordingly
- Educate patients and their families on managing their conditions and promoting wellness
- Collaborate with other healthcare professionals to coordinate patient care
- Document patient care activities accurately and timely
