As a Licensed Practical Nurse, you will play a vital role in providing exceptional patient care to our residents and patients. About the Role As a Licensed Practical Nurse at Springfield Center for Living and Rehabilitation, you will be responsible for providing direct patient care, administering medications, and collaborating with our interdisciplinary team of healthcare professionals to develop and implement individualized care plans. You will also have the opportunity to work with our staff to identify and address patient needs, respond to emergency situations, and maintain accurate and detailed records. Responsibilities Provide direct patient care, including administering medications, monitoring vital signs, and performing basic nursing skills Develop and implement individualized care plans in collaboration with the interdisciplinary care team Conduct regular rounds to assess patient status, identify potential issues, and make recommendations for improvement Collaborate with other healthcare team members to develop and implement effective care strategies Maintain accurate and detailed records of patient care, medication administration, and treatment plans Respond to emergency situations, such as code blues, fire alarms, or other emergency situations Participate in quality improvement initiatives to improve patient outcomes and care delivery Requirements Current licensure as a Licensed Practical Nurse (LPN) in the State of Vermont Strong clinical skills and knowledge of nursing principles and practices Excellent communication and interpersonal skills Ability to work effectively as part of an interdisciplinary team Strong problem-solving and critical thinking skills What We Offer At Springfield Center for Living and Rehabilitation, we are committed to providing a supportive and inclusive work environment that encourages professional growth and development.
