Day Shift / Night Responsibilities Provide direct patient care by performing assessments, vital signs, and physical examinations with attention to detail and accuracy. Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards. Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile dressing changes using aseptic techniques. Document all patient interactions accurately in electronic health record (EHR) systems like Epic or Cerner while maintaining strict compliance with HIPAA regulations. Monitor patients' conditions closely during acute care situations such as ICU or emergency room scenarios; respond promptly to changes in patient status. Collaborate with multidisciplinary teams including physicians, case managers, and caregivers to develop and implement individualized care plans. Support patients with specialized needs such as dementia care, Alzheimer's care, hospice services, or behavioral health management across various settings. Skills Proven experience working as an LPN in diverse environments such as hospitals (including Level I Trauma Centers), nursing homes, or outpatient clinics. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and documentation standards for medical records. Skilled in performing clinical procedures like venipuncture, phlebotomy, catheter care, airway management, and specimen collection/processing. Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for efficient medical documentation and data management. Expertise in critical care areas including ICU, emergency medicine, trauma centers, or post-acute care settings; experience with ventilator management preferred. Ability to work effectively with geriatric patients and individuals with disabilities or developmental challenges; experience in memory care or assisted living is highly valued. Knowledge of infection control practices and long-term care regulations to ensure safe patient environments. Excellent communication skills for caregiver interaction and client education on medication administration and health coaching. #
