Job Summary We are seeking a dynamic and compassionate T-Licensed Practical Nurse (LPN) to join our dedicated healthcare team. In this vital role, you will provide high-quality patient care across diverse settings, including long-term care, assisted living, and acute care environments. Your expertise will support patient assessments, medication administration, and care planning while ensuring compliance with healthcare regulations. Duties Perform comprehensive patient assessments, vital signs monitoring, and physical examinations to evaluate health status. Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards. Document all patient interactions accurately using Electronic Health Records (EHR) systems such as Epic or Cerner. Assist with medical procedures including catheterization, suturing, tube feeding, and sterile techniques. Collaborate with interdisciplinary teams to develop and implement personalized care plans for patients with complex needs such as dementia or Alzheimer's disease. Provide triage services by assessing urgent health concerns and coordinating appropriate interventions or referrals. Support infection control practices and adhere to HIPAA regulations to maintain patient confidentiality at all times. Experience Valid T-Licensed Practical Nurse certification with current licensure in the applicable state. Proven experience working in diverse healthcare environments such as long-term care facilities, hospitals (including ICU or trauma centers), hospice, or home care settings. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate medical documentation. Familiarity with medical coding systems including
