Job Summary We are seeking a dynamic and compassionate T-Licensed Practical Nurse (LPN) to join our dedicated healthcare team. Duties Perform comprehensive patient assessments, vital signs monitoring, and physical examinations to evaluate health status. Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards. Document all patient interactions accurately using Electronic Health Records (EHR) systems such as Epic or Cerner. Assist with medical procedures including catheterization, suturing, tube feeding, and sterile techniques. Collaborate with interdisciplinary teams to develop and implement personalized care plans for patients with complex needs such as dementia or Alzheimer's disease. Provide triage services by assessing urgent health concerns and coordinating appropriate interventions or referrals. Support infection control practices and adhere to HIPAA regulations to maintain patient confidentiality at all times. Experience Valid T-Licensed Practical Nurse certification with current licensure in the applicable state. Proven experience working in diverse healthcare environments such as long-term care facilities, hospitals (including ICU or trauma centers), hospice, or home care settings. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate medical documentation. Familiarity with medical coding systems including

for accurate record keeping and billing processes. Experience providing specialized care such as IV infusion therapy, pain management, dementia or memory care, and acute or emergency medicine. Knowledge of long-term care regulations and best practices for geriatric patient management. Strong communication skills for effective caregiver-patient interactions and team collaboration.
