The LPN Admissions Nurse is responsible for completing comprehensive start-of-care assessments for patients referred to home health or hospice services. This role evaluates patient eligibility, reviews clinical pieces of referral, obtains verbal CTI's, reviews documentation for accurate coding requirements, and ensures compliance with Medicare Conditions of Participation, Washington State regulations, and agency policies.
