Licensed Practical Nurse (LPN) Join the Rivers Edge Nursing & Rehab Team! We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team at Rivers Edge Nursing & Rehab in Muscoda, Wisconsin. As a Licensed Practical Nurse (LPN), you will play a vital role in providing high-quality care to our residents while working a dynamic and rewarding evening shift. About the
Role:
As an Evening Shift LPN, you will be responsible for: Providing direct patient care to residents in our long-term care facility, including administering medications, monitoring vital signs, and assisting with daily living activities Working collaboratively with our interdisciplinary team to develop and implement individualized care plans Maintaining accurate and detailed records of patient care and medication administration Providing emotional support and companionship to our residents and their families
Responsibilities:
Perform routine vital signs, such as temperature, blood pressure, and pulse Administer medications and treatments as prescribed by the physician Assist residents with daily living activities, such as bathing, dressing, and grooming Participate in care conferences and develop individualized care plans Maintain accurate and detailed records of patient care and medication administration Provide emotional support and companionship to residents and their families
Requirements:
Current LPN license in the state of Wisconsin High school diploma or equivalent required; Associate's degree in nursing or related field preferred Previous experience in a long-term care setting preferred
