Weekend Requirement:
Every other weekend Position Overview Licensed Practical Nurse (LPN) to provide quality nursing care to residents in a long-term care setting. The LPN will be responsible for medication administration, monitoring residents' conditions, communicating changes to physicians and families, collaborating with CNAs, and maintaining accurate and timely clinical documentation. The ideal candidate will demonstrate excellent clinical judgment, attention to detail, communication skills, and a strong commitment to resident safety and quality of care. Key Responsibilities Assess residents' health status and monitor for changes in condition. Administer medications safely and accurately while following the Five Rights of Medication Administration . Report changes in residents' condition promptly to the charge nurse and appropriate healthcare providers. Notify physicians and other members of the healthcare team regarding significant changes in resident status. Communicate relevant changes in condition with residents' family members as appropriate. Report any suspected or alleged abuse, neglect, or misappropriation immediately to the Director of Nursing and Administrator in accordance with facility policies and applicable regulations. Assist CNAs with resident care, call lights, and other appropriate responsibilities. Required Qualifications Current and active Wisconsin LPN license . Current CPR/BLS certification required. Previous long-term care or related nursing experience preferred. 1+ year of related experience preferred , although candidates with less than one year may be considered. #HC1
