Job Description

Job Overview We are seeking a dedicated Licensed Practical Nurse (LPN) specializing in Long Term Care to join our healthcare team. The ideal candidate will possess a strong commitment to providing compassionate, patient-centered care within a skilled nursing or assisted living environment. This role involves delivering high-quality nursing services to residents, supporting their health and well-being, and ensuring compliance with all long-term care regulations. The LPN will work collaboratively with interdisciplinary teams to promote optimal health outcomes and enhance the quality of life for our residents. Responsibilities Perform comprehensive patient assessments, monitor vital signs, and document clinical information accurately using Electronic Health Record (EHR) systems. Administer medications, including injections and IV infusions, following prescribed protocols while adhering to HIPAA and clinical confidentiality policies. Assist with activities of daily living (ADLs), such as bathing, dressing, and mobility support, ensuring respectful and dignified care. Manage wound care, including dressing changes and infection control procedures, with attention to aseptic techniques. Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans tailored to residents' needs. Provide specialized dementia care, memory support, and Alzheimer's care to residents with cognitive impairments. Support discharge planning, case management, and coordinate with families for comprehensive long-term care services. Skills Proven experience in long-term care settings such as nursing homes or skilled nursing facilities; experience in assisted living is a plus. Proficiency in EMR/EHR systems such as Cerner or eClinicalWorks for accurate medical documentation. Strong knowledge of
ICD-9/10
coding, CPT coding, and medical terminology relevant to long-term care. Skilled in IV insertion, catheterization, tube feeding management, and basic life support (BLS). Experience working with geriatric patients, individuals with developmental disabilities, and those requiring hospice or palliative care. Familiarity with acute pain management, infection control practices, and emergency response procedures including CPR. Excellent caregiver communication skills to effectively educate residents and coordinate with families while maintaining professional boundaries. Up to $1,893.00 per week
