The APS benefit includes asthma self-management education, in-home environmental trigger assessments, and asthma home visiting services. The ideal candidate will work with clients diagnosed with asthma, providing education, assessments, and remediation services to improve asthma control and overall health outcomes.

APS Responsibilities:

1. Provide APS services to clients with an asthma diagnosis, including: a. Asthma self-management education b. In-home environmental trigger assessments c. Asthma education d. Environmental asthma trigger assessment and remediation e. Conduct asthma home visiting services, including assessments and education f. Use the Asthma Control Test (ACT) to assess client asthma control g. Develop and implement individualized care plans h. Collaborate with healthcare providers and other stakeholders to ensure comprehensive care i. Maintain accurate records and documentation

ECM Responsibilities:

a. Conduct patient assessments to identify health and social needs. b. Develop personalized care plans with patients, families, and healthcare providers. c. Provide education and support on chronic disease management, healthy behaviors, and self-care. d. Facilitate access to community resources, services, and referrals. e. Coordinate care transitions and communicate with healthcare providers. f. Regular home visits and phone follow-ups should be conducted to monitor progress. g. Document patient interactions and care plans in the electronic health record within 24 hours and close all encounters. h. Collaborate with interdisciplinary teams to address patient needs and improve health outcomes. i. Identify and address social determinants of health and health disparities. j. Provide cultural competency and linguistically appropriate services. k. Maintain patient confidentiality and adhere to HIPAA guidelines. l. Minimum caseload of 40 clients, all must be seen each month. m. CHW minimum productivity is expected to be 80% clinical (patient billable encounters) and 20% Non-productive (driving/documentation/outreach, etc.). n. Schedule 75% of clients before the 15th of each month. o. Conduct outreach to increase community awareness of the ECM program. p. Attend regular sta[ meetings, training sessions, and other meetings as requested.

NON-ESSENTIAL DUTIES AND RESPONSIBILITIES

Performs other duties as assigned. Ability to work independently and be self-directed and flexible. Ability to perform functions with minimal supervision. Ability to work at a high-volume level of accuracy.

Requirements:

1. Medical Assistant certification 2. Completion of an approved training program (e.g., California Department of Public Health's Asthma Management Academy) 3. Minimum of 16 hours of face-to-face client contact focused on asthma management and prevention 4. Four hours annually of continuing education on asthma. 5. A bachelor's degree in psychology, social work, or a related field. 6. Community-based work, also known as in vivo work, requires proper safety precautions. 7. Certification as a CHW (CCHW) or willingness to obtain within 18 months. 8. CPI and CPR within 60 days of hire. 9. Strong communication, interpersonal, and cultural competency skills. 10. Ability to work independently and as part of a team. 11. Basic computer skills and familiarity with electronic health records. 12. Valid driver's license and reliable transportation (if applicable). Preferred Qualifications: 1. Certified as a CHW (CCHW) 2. Experience with asthma management and prevention services 3. Knowledge of environmental health and asthma triggers 4. Bilingual (Spanish/English) or ability to communicate with diverse populations 5. Experience working with diverse populations and communities. 6. Knowledge of community resources and services. 7. Familiarity with care management software and technology. 8. Bilingual or multilingual skills.
