In-Office Call Center About the Position We are seeking an experienced, compassionate, and highly professional Medical Assistant to join our growing Remote Patient Monitoring (RPM) team. This is a patient-facing position in which you will communicate with patients primarily by telephone, review remote monitoring activity, conduct structured health assessments, encourage patient compliance, document interactions, and escalate clinical concerns according to established protocols. The ideal candidate is comfortable speaking with patients throughout the day and knows how to combine professionalism with empathy. You should be organized, dependable, detail-oriented, and capable of building rapport with older adults and patients managing chronic health conditions. Key Responsibilities As a Medical Assistant on our RPM/CCM team, your responsibilities will include: Conduct scheduled telephone outreach and wellness calls with patients enrolled in RPM programs Review patients' blood pressure readings and remote monitoring activity Identify missing, inconsistent, or potentially abnormal readings and follow established escalation protocols Encourage patients to remain compliant with prescribed monitoring schedules Conduct structured wellness questionnaires and patient assessments Perform fall-risk, gait, and balance assessments and document changes or concerns Conduct medication adherence reviews and confirm patients understand and are following their medication regimens Ask patients about changes in symptoms, general health, recent falls, hospitalizations, emergency room visits, and other relevant health events Provide appropriate reminders regarding medications, monitoring, appointments, and other elements of the patient's care plan Accurately document all patient interactions in the appropriate electronic medical record (EMR/EHR) or patient management platform Escalate concerning readings, symptoms, or changes in patient condition to the appropriate clinical provider according to established protocols Coordinate with physicians, nurse practitioners, physician assistants, and other members of the clinical care team Maintain patient confidentiality and follow HIPAA and organizational compliance requirements Maintain an organized daily patient outreach schedule and complete assigned patient contacts in a timely manner Required Qualifications Previous Medical Assistant experience Excellent telephone communication and interpersonal skills Strong organizational and documentation skills Ability to communicate professionally and compassionately with patients Comfortable working with older adults and patients with chronic medical conditions Comfortable using computers, EMR/EHR systems, and multiple software platforms Ability to follow established clinical protocols and recognize when information needs to be escalated Reliable, punctual, and capable of managing a structured daily workload Strong attention to detail and accuracy Preferred Experience Preference will be given to candidates with previous experience in one or more of the following areas: Remote Patient Monitoring (RPM) Chronic Care Management (CCM) Telehealth Blood pressure monitoring Patient telephone outreach Medicare patient populations Wellness assessments Fall-risk and balance assessments Medication adherence/compliance EMR/EHR documentation Chronic disease management Experience working with patients managing conditions such as hypertension, diabetes, cardiovascular disease, and other chronic illnesses is especially valuable.
