This role works closely with providers and nursing staff to monitor patient-reported data, coordinate care activities, and ensure accurate documentation in compliance with CMS guidelines. The ideal candidate is detail-oriented, tech-savvy, and passionate about proactive, preventive care. Key Responsibilities Conduct outbound and inbound patient calls to support RPM and CCM programs. Assist with onboarding patients into remote monitoring programs, including device education and troubleshooting. Collect and document vital sign readings (e.g., blood pressure, glucose, weight, pulse oximetry) from RPM platforms. Identify abnormal readings or patient concerns and escalate to the nurse or provider per protocol. Support development and updating of individualized care plans under clinical supervision. Perform medication reconciliation and review adherence with patients. Schedule follow-up appointments and coordinate referrals as needed. Close care gaps by assisting with preventive screening reminders and health maintenance outreach. Ensure timely, accurate documentation in the electronic health record (EHR). Maintain compliance with HIPAA and CMS requirements for RPM and CCM services. Proficiency with EHR systems and digital health platforms. Basic understanding of chronic disease processes and vital sign parameters. Ability to follow clinical protocols and escalate concerns appropriately. Strong organizational and time-management skills in a remote environment

Pay:

$12.00 - $15.00 per hour
