‑D, HCS‑H, COC, or CRC Description Position Summary:
The Coding Specialist is responsible for accurately assigning ICD‑10‑CM diagnosis codes for home health and hospice encounters. This role ensures coding accuracy, regulatory compliance, and complete documentation to support OASIS submission, billing, and claims processing. The specialist works closely with clinical, physician, and revenue cycle teams to support quality patient care and compliant reimbursement. Key Responsibilities Coding & Documentation Assign and sequence accurate ICD‑10‑CM diagnosis codes for home care and hospice encounters. Review medical records and OASIS assessments to identify conditions requiring coding and all data elements needed for OASIS and claims submission. Ensure all codes and abstracted data are complete, accurate, and ready for OASIS submission, billing interfaces, and claims processing. Compliance & Coding Standards Apply Official ICD‑10‑CM Coding Guidelines, Coding Clinic guidance, CMS regulations, and internal organizational policies. Maintain compliance with national provider query standards when addressing incomplete, unclear, or nonspecific documentation. Collaboration & Communication Work collaboratively with physicians and clinical staff to obtain necessary documentation for complete and accurate coding. Communicate with and advise home care clinical and revenue cycle teams on diagnosis coding and PDGM assignments. Serve as a coding resource to support documentation quality and reimbursement accuracy. Productivity & Quality Consistently meet established productivity standards. Maintain a coding accuracy rate of 96% or higher. Additional Duties Perform other coding, data abstraction, data entry, medical record, or revenue cycle-related tasks as assigned. Performance Expectations & Professional Standards Adhere to Service Excellence Standards and expectations of Chesapeake Regional Medical Center. Demonstrate professional, courteous, and respectful communication with patients, visitors, physicians, and coworkers. Exhibit strong judgment, teamwork, and collaboration to continuously improve work quality. Maintain flexibility in work scheduling when needed to support patient care. Present a professional image through appropriate attire, behavior, and etiquette. Attend required hospital orientations, meetings, and in‑service training.Additional Skills & Qualifications Education Preferred:

Pay and Benefits The pay range for this position is $23.00 - $27.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
