Job Description

Director of Medical Records (LPN) Are you a detail-oriented Licensed Practical Nurse (LPN) with experience in medical records and healthcare compliance? We are seeking a Director of Medical Records to lead and maintain the integrity, accuracy, confidentiality, and regulatory compliance of our medical records and health information management program. This position plays a critical role in supporting resident care, physician credentialing, survey readiness, and compliance with federal and state regulations.
What You'll Do:
Oversee the daily operations of the Medical Records Department. Ensure resident medical records are complete, accurate, organized, and maintained according to company policies and regulatory requirements. Review admission, discharge, and death records for completeness and compliance. Audit medical records for accuracy, coding, signatures, and required documentation prior to filing. Manage the collection, indexing, coding, storage, retrieval, and retention of resident records. Ensure ICD coding and health information practices comply with current regulations and standards. Maintain active and inactive medical records in accordance with established record retention policies. Respond to authorized requests for medical records from physicians, government agencies, insurance providers, and other approved entities. Compliance & Credentialing Ensure compliance with all applicable federal, state, and local regulations governing health information management. Coordinate and maintain credentialing records for medical staff and providers in accordance with regulatory requirements. Advise facility leadership on medical records regulations, compliance issues, and best practices. Maintain confidentiality and safeguard protected health information in accordance with privacy regulations. Prepare records and documentation for audits, surveys, legal proceedings, and regulatory reviews as needed. Collaboration & Support Partner with nursing, clinical, administrative, and physician teams to ensure timely and accurate documentation. Monitor and resolve incomplete records by coordinating with appropriate departments. Assist with incident and accident documentation processes. Participate in departmental meetings, training programs, and continuing education activities. Provide exceptional customer service to residents, families, staff, and external partners. Qualifications Required Graduate of an accredited Nursing program. Current, active, and unrestricted Licensed Practical Nurse (LPN) license. Minimum of 2 years of medical records experience in a healthcare setting. Knowledge of medical terminology, health information management practices, and healthcare regulations. Strong computer skills, including electronic medical records (EMR) systems and data management. Ability to prioritize tasks, work independently, and maintain strict confidentiality. Excellent communication and interpersonal skills. Preferred Certification or formal training in Electronic Medical Records (EMR). Experience in long-term care, skilled nursing, rehabilitation, or post-acute healthcare settings. Familiarity with physician credentialing processes and healthcare compliance programs. Why Join Us? Pay:
$30.00 per hour Expected hours: 40.0 per week
