| Lead Innovative, Community-Based Senior Care | Rochester, NY Independent Living for Seniors - 3.5 Rochester, NY Job Details Full-time $250,000 - $300,000 a year 21 hours ago Qualifications Team leadership Full Job Description Rochester Regional Health is seeking an experienced physician leader to serve as Medical Director for ElderONE , the region's Program of All-Inclusive Care for the Elderly (PACE). ElderONE provides comprehensive, coordinated care that helps frail older adults safely remain in their homes and communities. The Medical Director oversees clinical quality, regulatory compliance, and interdisciplinary team performance while also providing direct patient care. This leader partners closely with operational leadership to drive program growth, quality outcomes, and participant satisfaction.
Key Responsibilities:
Provide clinical leadership for all Elder
ONE/PACE
sites Lead and mentor interdisciplinary care teams Ensure compliance with CMS and
NYSDOH PACE
regulations Oversee care planning, quality initiatives, and clinical standards Contribute to strategic planning, program operations, and performance improvement
Qualifications:
M.D. or D.O., board certified in Geriatrics, Internal Medicine, or Family Medicine (Geriatrics preferred) Eligible for NY medical licensure Minimum 5 years of relevant clinical experience; leadership experience strongly preferred Strong communication, collaboration, and team leadership skills About Elder
ONE / PACE
ElderONE, sponsored by Rochester Regional Health, is the oldest PACE program in New York State. PACE provides comprehensive medical and social services to eligible older adults through an interdisciplinary team approach, enabling participants to remain safely at home rather than entering a nursing facility. Services include primary and specialty care, rehabilitation, transportation, social engagement, and 24/7 clinical support. About Rochester Regional Health Rochester Regional Health is one of the largest integrated health systems in New York State, serving communities across Western New York and the Finger Lakes. The system includes five hospitals, over 300 practice locations, leading home-based and community care programs, and a shared commitment to clinical excellence, innovation, and compassionate service.

PHYSICAL REQUIREMENTS
L - Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly; requires occasional walking, standing or squatting. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. 14607 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
