In this role, you will document patient encounters in real time, including histories, exams, procedures, and treatment plans within the electronic health record. You will help manage orders, results, and follow-up documentation, ensuring accuracy and completeness while maintaining strict patient confidentiality. Working closely with physicians and clinical teams, you'll contribute to efficient workflows and high-quality care, gaining valuable exposure to emergency and acute care environments in a supportive, collaborative culture. Responsibilities Document patient histories, exams, and treatment plans in real time during clinical encounters Enter orders, lab and imaging requests, and prescriptions as directed by clinicians Review and update electronic health records for accuracy and completeness Assist clinicians with chart preparation and post-visit documentation follow-up Track test results and consultations, ensuring timely documentation and communication Maintain patient confidentiality and comply with HIPAA and organizational policies Collaborate with physicians, nurses, and staff to support efficient patient flow Use EHR systems, templates, and medical terminology accurately Identify documentation gaps and seek clarification from providers Participate in training and process improvement to enhance documentation quality Required Skills Medical terminology Electronic health records (EHR) documentation Typing and data entry Clinical documentation standards HIPAA and patient privacy compliance Order entry workflows Basic anatomy and physiology knowledge Chart review and audit Time management in clinical settings Professional written communication
