QMAP (Qualified Medication Administration Person) Job Description Position Summary The Qualified Medication Administration Person (QMAP) is responsible for safely administering medications in accordance with Colorado regulations while providing compassionate daily care to residents. This position helps maintain a safe, clean, and comfortable living environment by assisting residents with activities of daily living, monitoring changes in condition, and supporting the overall well-being of each resident. Key Responsibilities Administer medications safely and accurately according to physician orders, Colorado QMAP regulations, and facility policies. Follow the six rights of medication administration and document all medications appropriately. Monitor residents for medication effectiveness, side effects, and adverse reactions, reporting concerns promptly. Perform and document vital signs as directed. Maintain medication security, including proper handling and counting of controlled substances. Reorder medications or notify appropriate staff when refills are needed. Report medication refusals, medication errors, and incidents according to facility procedures. Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, mobility, and oral hygiene. Observe and report changes in residents' physical or mental condition to the appropriate supervisor. Document resident care accurately and promptly. Assist with resident admissions, discharges, and transfers. Maintain clean resident rooms, common areas, linens, wheelchairs, walkers, and other equipment. Assist with meal service, dining room setup, and cleanup. Participate in resident activities and outings as requested. Maintain resident privacy and confidentiality in accordance with HIPAA and facility policies. Follow all infection control, fire safety, emergency preparedness, and workplace safety procedures. Keep work areas clean, organized, and stocked. Communicate effectively with coworkers during shift changes to ensure continuity of care. Perform other duties as assigned. Qualifications Current Colorado QMAP certification required. Current First Aid and CPR certification preferred. High school diploma or GED preferred. Previous experience in assisted living, long-term care, or healthcare preferred. Ability to read, write, speak, and understand English. Basic computer skills, including Microsoft Office and electronic documentation systems. Strong communication, organizational, and interpersonal skills. Ability to maintain confidentiality and provide compassionate resident-centered care. Physical Requirements Ability to stand and walk for extended periods. Frequently lift, move, or assist residents and equipment weighing up to 30 pounds. Ability to bend, stoop, reach, push, pull, and perform repetitive movements. Ability to safely assist residents with mobility and transfers. Ability to work weekends, holidays, and varying shifts as scheduled. Work Environment Assisted living community providing 24-hour resident care. Frequent interaction with residents, families, visitors, and healthcare professionals. Exposure to cleaning chemicals, bodily fluids, and infectious diseases while following appropriate safety precautions. May be required to respond to emergency situations outside of normal work hours.
