Job Description

Minnesota Greenleaf Medication Aide/Caregiver Reports to:
Administrator/DON Job Classification :
Hourly wage (nonexempt)
Minimum Qualifications:
The Resident Care Provider is responsible for providing compassionate, person-centered care and support to residents living in an assisted living and memory care setting. This position assists residents with activities of daily living, promotes independence and dignity, provides meaningful engagement, and helps maintain a safe, comfortable, and welcoming environment. The Resident Care Provider plays an essential role in supporting residents with physical, emotional, social, and cognitive needs while following individualized service plans, facility policies, Minnesota assisted living regulations, and direction from nursing and leadership staff. Essential Duties and Responsibilities Resident Care and Personal Assistance Provide direct care and assistance to residents according to their individualized service plans. Assist residents with activities of daily living (ADLs), including: Bathing and personal hygiene Dressing and grooming Toileting and incontinence care Transfers, mobility, and ambulation Eating and hydration Other individualized care needs Encourage and support residents to maintain independence whenever safely possible. Provide care in a respectful, dignified, compassionate, and person-centered manner. Observe residents for changes in physical condition, behavior, mood, cognition, appetite, mobility, or overall well-being. Promptly report changes in resident condition to the nurse or appropriate supervisor. Respond appropriately to resident emergencies and follow facility emergency procedures. Memory Care Responsibilities Provide specialized care and support to residents living with dementia, Alzheimer's disease, and other cognitive impairments. Use person-centered and dementia-informed approaches when interacting with residents. Provide reassurance, redirection, validation, and emotional support when residents experience confusion, anxiety, agitation, or distress. Promote a calm, structured, safe, and supportive environment. Assist with meaningful activities and engagement appropriate to each resident's interests and abilities. Monitor residents for safety concerns, including wandering, falls, behavioral changes, and other risks. Maintain awareness of memory care security procedures and assist in protecting residents from elopement or unsafe situations. Treat each resident as an individual and respect their personal history, preferences, routines, and abilities. Medication Administration Assist with medication-related duties only as permitted by Minnesota law, facility policy, training, and delegation. Medication administration duties may be performed by properly trained and authorized staff. Observe and report medication concerns, refusals, side effects, or changes in resident condition to the appropriate nurse or supervisor. Follow infection prevention and control procedures. Assist with health monitoring and other delegated tasks within the scope of training and competency. Documentation and Communication Complete required documentation accurately, timely, and thoroughly. Document care and services provided according to facility policy. Communicate effectively with nurses, supervisors, coworkers, residents, families, and other members of the interdisciplinary team. Report incidents, accidents, injuries, falls, unusual occurrences, and changes in resident condition according to facility policy. Maintain resident confidentiality and comply with HIPAA and privacy requirements. Participate in shift reports and communicate important resident information to incoming staff. Customer Service and Teamwork Create a positive, welcoming, and respectful environment for residents, families, visitors, and coworkers. Communicate professionally and compassionately with resident family members. Work collaboratively with nursing, dietary, housekeeping, activities, maintenance, and leadership staff. Demonstrate flexibility and willingness to assist coworkers and respond to changing resident needs. Promote a positive workplace culture built on teamwork, respect, accountability, and compassion. Maintain professional boundaries at all times. Safety and Facility Responsibilities Follow all facility policies and procedures. Maintain a clean, safe, and organized resident environment. Follow proper lifting, transfer, and body mechanics procedures. Use personal protective equipment and follow infection control practices. Identify and report safety hazards immediately. Participate in emergency drills and follow emergency preparedness procedures. Assist with light housekeeping, laundry, meal service, and other resident-related duties as assigned. Qualifications Required Previous caregiving, healthcare, assisted living, or memory care experience preferred but not always required. Must successfully complete required orientation, training, competency evaluations, and continuing education. Ability to follow written and verbal instructions. Demonstrated compassion, patience, reliability, and professionalism. Ability to work independently while also functioning effectively as part of a team. Must meet applicable background study and employment eligibility requirements. Preferred Experience working with older adults or individuals living with dementia. Nursing assistant, home health aide, or caregiving experience. Training in dementia care, person-centered care, and behavioral support techniques. CPR and First Aid certification, as required by facility policy. Physical Requirements The physical demands described below are representative of those necessary to perform the essential functions of this position: Ability to stand and walk for extended periods. Ability to assist residents with mobility and transfers. Ability to bend, stoop, reach, push, pull, and lift as necessary. Ability to safely perform resident transfers using appropriate equipment and techniques. Ability to respond quickly to resident emergencies. Ability to perform repetitive physical tasks associated with direct resident care. Work Environment This position works in an assisted living and memory care environment. The employee may be exposed to residents with cognitive impairment, medical conditions, behavioral symptoms, and occasional emergency situations. The position requires flexibility, patience, sound judgment, and the ability to remain calm and professional in stressful situations.
