Medication History Technician

- This position entails assisting with the medication reconciliation process by interviewing patients and/or family members and contacting pharmacies, physician offices, nursing home staff, and using any other available resources to obtain an accurate medication and allergy history.
This best possible preadmission medication list obtained by the Medication Reconciliation Technician will be used to update the existing medication list documented in the electronic medical record and utilized by healthcare providers in the performance of medication reconciliation throughout the continuum of care . In addition, the position will require serving as a resource for the pharmacist and assisting nursing and physicians with medication related requests, and to encourage and facilitate the use of the Lobby Pharmacy by patients for the filling of discharge prescriptions. The technician will communicate pertinent information (i.e. medications that need to be clarified with the physician and/or other healthcare provider) to the supervising pharmacist.
- Salary Range:
- $23.50-$30.25 An hour _The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations._

Job Requirements:

Education:
Must fulfill the requirements to be Certified through PTCB or NHA +
Experience:
At least one year of pharmacy experience; (3-5 years preferred); previous experience in community pharmacy preferred +
License/Certification Required:
Must be board certified by PTCB or NHA upon hire or transfer into the Tech IV role. Must maintain certification throughout employment +
Skills:
