Job Responsibilities:

Take and record vital signs, including blood pressure, pulse, respiration, temperature, oxygen saturation, and weight. Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, feeding, and mobility. Assist with patient transfers, ambulation, positioning, and maintaining patient safety. Observe patients for changes in physical condition, behavior, pain, mental status, or other symptoms and promptly report concerns to the RN/LPN or other healthcare provider. Document patient care, observations, medications, treatments, and changes in condition accurately in the medical record. Assist nursing staff with delegated patient-care tasks within the scope of training and competency. Maintain a clean, safe, sanitary, and organized patient-care environment. Track patient medications and assist with medication-related follow-up activities. Perform other patient-care and healthcare-support duties as delegated by the supervising nurse.
