Summary:
The Medication Discharge Tech (MDT) is responsible for identifying patients with specific disease states or medications that are likely to result in readmission back to the facility. They will identify discharge medications that may require financial assistance. They will also address any medication accessibility issues that could present barriers upon discharge. The MDT will try to resolve any medication issues that need to be addressed before discharge to ensure a safe and timely discharge process. The MDT will engage with these identified patients and offer Ballad Health Pharmacy services to fill their prescriptions, identify any issues or financial needs, and deliver bedside. The MDT serves as the main point of contact between patients, floor staff, and the retail pharmacy. In this position, the MDT manages the bedside delivery process from the identification of the patient and discharge medications to having the meds in the patient's possession upon discharge. The MDT will enhance communication with staff, improve patient care and satisfaction, grow clinical staff buy-in, increase discharge prescription capture rates, and deliver discharge medications to patient bedside. The MDT will also be cross trained to provide support to the retail pharmacy, as needed. Retail pharmacy duties will include running test claims, obtaining verbal orders from the provider, obtaining and giving transfers, and initiating prior authorizations. The MDT will coordinate patient care for discharge prescriptions with a multi-disciplinary team and communicate directly with patients and caregivers to ensure timely and accurate filling of prescriptions and obtain any documentation as required. The MDT must manage patient complexity and multiple patients with diverse needs as well as develop strong interpersonal relationships with the patient care team.
Requirements:
Team member must be nationally certified with an active state license. If the team member is not certified or licensed, they will need to obtain these credentials within three months of employment with Ballad.
Licenses and Certifications:
National Certification within 3 months of hire. such as PTCB or NCH, and State Licensure within 3 months of hire.
