Health Connect America, Inc. seeks an AL professional to support delivery of behavioral health, counseling, and therapeutic services to children, adolescents, adults, and families. In this role, you'll help coordinate in home, in office, and community based mental and behavioral healthcare, ensuring services are timely, well documented, and client centered. You will collaborate with multidisciplinary clinicians, community partners, and agencies, assist with referrals and scheduling, maintain accurate records, and uphold compliance and confidentiality standards while supporting our mission of accessible, compassionate care across local communities. Responsibilities Support coordination of behavioral health and counseling services for children, adolescents, adults, and families. Assist with client intake, referrals, and scheduling for in-home, in-office, and community-based services. Maintain accurate, timely documentation and records in electronic health systems. Communicate with clients, families, community partners, and agencies to support service delivery. Ensure adherence to confidentiality, HIPAA, and agency policies and procedures. Support multidisciplinary clinicians with caseload management and follow-up tasks. Help track service authorizations, appointments, and basic outcome or utilization data. Participate in team meetings and collaborate to improve access and quality of care.
