Job Description

Position 1: 3p-11p Week 1: Wed, Sat, Sun; Week 2:
Mon, Fri Position Definition:
Provide consumers with support and/or assistance with ADLs (basic activities for daily life including but not limited to personal hygiene, bladder and bowel control, dressing, taking medication, ambulating etc.) and IADLS (complex activities including but not limited to shopping, doing laundry, making and keeping appointments, managing finances, using a prosthetic device, etc.) as needed to promote wellness and independence.
Job Responsibilities:
Assist in the development and implementation of consumer support plans; maintain and update documentation in consumers individual records; assure consumer safety and well-being, respond immediately to emergency situations affecting consumers health or safety; report any signs of consumer decompensation to the administrator, complete menu planning, grocery shopping, meal preparation, daily and weekly housekeeping and laundry tasks; provide consumer transportation; follow and abide by confidentiality policies; perform related duties as needed to assist consumers with ADLs and IADLs; initiate social/recreational activities in-house and in the community and follow through with the same.
