OBJECTIVE:
Under the direct supervision of the Program Manager. Will organize and coordinate Case Management services for residential youth in the Crisis Stabilization program. Will meet with CYFD weekly to coordinate exit plans for clients. Will be an onsite supervisor overseeing day-to-day programming, scheduling, intakes, and other related duties as assigned.
MINIMUM QUALIFICATIONS
  • Must be at least 21 years old.
Bachelor's degree in human service field from an accredited university and have one-year relevant experience with the target population; or an associate degree and have a minimum of two (2) years of experience working with the target population; or a high school graduate or have a General Educational Development (GED) test and a minimum of five (5) years of experience working with the target population, and a minimum of one (1) year supervisory experience.
  • Extensive knowledge and experience working in a residential setting.
  • Extensive knowledge of community resources and ability to work efficiently with public and private agency professionals.
  • Broad knowledge of assessment, service planning and service coordination in working with adolescents and families.
  • Ability to establish an effective professional manner in all dealings with clients and the public.
  • Ability to exhibit emotional maturity and good judgment, to include high moral standards and proper oral/written communication skills.
  • Must be dependable and flexible regarding working hours.
  • Ability to follow and carry out written and oral instructions, quickly and accurately.
  • Must be able to use discretion in all facets of work-related situations.
  • Possess the ability to meet all deadlines while multi-tasking.
  • Broad knowledge and experience in working with adolescents and their families.
  • Must keep abreast of modern techniques and methods used in working with youth in a residential setting.
DUTIES AND RESPONSIBILITIES
(Not Inclusive):
  • Will complete all referrals, interviews, and intakes for youth entering residential services.
  • Will assign staff to work with certain youth in the facility.
  • Will hold primary responsibility for coordinating ALL services and clinical care for residential youth, including assisting with referrals, and accessing other needed services.
  • Will provide life and social skills training that address goals specifically in budgeting, resume writing, job interviewing skills, etc.
  • Will provide Case Management Services consisting of a variety of interventions.
  • Will provide Case Management Services including assistance with identifying and coordinating services and support identified in an individual's service plan; supporting an individual and family in crisis situations; and providing individual interventions to develop or enhance an individual's ability to make informed and independent choices.
  • Will work with the assigned therapist to develop a comprehensive safety plan for each client.
  • Will review and approve notes from residential staff.
  • Will coordinate and facilitate treatment team meetings as indicated in the service plan.
  • Will monitor and evaluate the provision of client services on an ongoing basis.
  • Will provide maintenance services for clients that have stabilized and are not in need of intensive services.
  • Will maintain comprehensive client records and generate reports as required.
  • Will perform other duties that directly promote client well-being and self-sufficiency.
  • Will serve in a day-to-day supervisory role overseeing both Residential Staff and youth in program.
  • Supervisory duties will include, but are not limited to; overseeing staff schedule, responding to day-to-day crisis calls, advising the Program Manager regarding technical and/or clinical incidents that need to be addressed, ensure policies and procedures are being followed, participate and/or facilitate staff meetings, ensure reports and files are completed and submitted in a timely manner, consult with and take direction from Program Manager.

JOB CHARACTERISTICS/PHYSICAL DEMANDS
This is a professional position which requires organizational and communication skills, as well as judgment, tact and diplomacy in contacts with Youth Development, Inc. employees, community residents, parents and local government agencies. Strict adherence is required to all regulations concerning confidentiality. Good general health. Physical will be required at time of hire. Background investigation to include fingerprinting will be required. Frequent travel is required. Valid driver's license and proof of satisfactory car insurance are required. Must be able to perform essential job duties with or without reasonable accommodation. Youth Development, Inc. is an at-will employer.
