Job Description

The QMHP is responsible for delivering goal-directed training to enable individuals diagnosed with a serious mental illness to achieve and maintain community stability and independence in the most appropriate, least restrictive environment. Skill-building services focus on functional skills and appropriate behavior related to the individual's health and safety; activities of daily living and use of community resources; assistance with medication management; and monitoring of health, nutrition, and physical condition. Services provided are based on needs identified during the assessment process, are guided by an Individual Service Plan and take place in the individual's home and community.
