Job Description

Nature of Work:
Position responsible for providing direct and supportive care to clients within the Assertive Community Treatment (ACT) program. ACT is a community-based mobile mental health treatment, rehabilitation and support team serving adults with serious behavioral health and substance abuse illnesses. Individuals served by the ACT team are typically those for whom traditional mental health services have been ineffective or inadequate. Work hours require flexibility, with day, evening, weekend and holiday periods needing coverage to address specific client needs.
Responsibilities:
Serves as a member of the ACT team attending all designated meeting. Provides direct care to ACT consumers to include: Case management functions Crisis de-escalation Transportation Provides and assist with personal care and ADL's when necessary Provides supportive counseling services individual and in group format within scope of training and competencies. Facilitates social and recreational activities Administration of medication with completion of appropriate training Documents all service activities including collateral contacts in a complete accurate and timely manner according to DHHR licensing regulations and Westbrook policy. Participates in on call schedule as member of team Monitors clients safety needs, including physical, medical, behavioral and psychiatric. Assist with Continuous Quality Improvement Program Maintain a level of attendance consistent with Westbrook Health Services policy. Work with staff, clients and others in a professional, ethical and respectful manner. Works to ensure a safe environment for staff, consumers and associates by following Westbrook's safety policies.
