•Responsible for planning, organizing, and administering recreational therapy programs tailored to the needs of patients receiving home health services. Assess patient abilities, interests, and needs to develop individualized therapeutic recreation plans that promote physical, emotional, and social well-being. Utilize a variety of activitiesincluding arts and crafts, music, games, exercise, and community outingsto enhance patient engagement and quality of life. Collaborate with nurses, therapists, social workers, and family members to coordinate care and integrate recreational therapy into each patients comprehensive treatment plan. Document patient progress, evaluate program effectiveness, and make recommendations for adjustments to meet changing needs. Educate patients and families on the benefits of recreational therapy and provide guidance on community resources and at-home activities. Maintain compliance with federal, state, and company regulations, uphold ethical standards, and ensure safe and supportive environments during all therapeutic activities. Participate in team meetings, training sessions, and continuing education to stay current with best practices in recreational therapy for home health settings.
