Nurse Practitioner (NP) Are you a passionate NP seeking to provide compassionate care to a deserving senior population? Our facility is seeking a dedicated and experienced Nurse Practitioner (NP) to join our team and deliver high-quality primary care to our residents. In this fulfilling role, you'll play a vital role in ensuring the health and well-being of our residents, working collaboratively with our interdisciplinary team to create personalized care plans.Responsibilities:
Conduct comprehensive physical examinations, diagnose and treat acute and chronic illnesses in a geriatric population. Develop and implement individualized care plans that address residents' medical, physical, and psychological needs. Prescribe medications, order and interpret diagnostic tests, and perform procedures within your scope of practice. Collaborate with physicians, nurses, social workers, and therapists to ensure coordinated care. Educate residents and families about health conditions and treatment options. Advocate for residents' well-being and ensure they receive appropriate care. Document patient care accurately and thoroughly in the electronic health record (EHR).Qualifications:
Active certification as a Nurse Practitioner (NP) with a focus on geriatrics. Minimum 2 years of clinical experience in a geriatric setting (nursing home, long-term care, or geriatric clinic). Strong clinical skills in geriatric assessment, diagnosis, and treatment. Excellent communication, collaboration, and interpersonal skills. Compassionate and patient-centered approach to care. Proficient in EHR documentation.We Offer:
