The ideal candidate is committed to providing high-quality, patient-centered care and is comfortable working collaboratively with medical, behavioral health, and administrative staff. Position Summary The Family Nurse Practitioner will provide comprehensive primary care services to adult clients, including assessment, diagnosis, treatment, medication management, preventative care, health education, and ongoing management of acute and chronic medical conditions. The FNP will work as part of an integrated care team to support continuity of care and improve overall health outcomes. Responsibilities Conduct comprehensive medical evaluations, health histories, and physical examinations. Diagnose and treat acute and chronic medical conditions within the FNP's scope of practice. Develop individualized treatment and care plans. Prescribe medications and therapeutic treatments in accordance with applicable laws, regulations, and clinical standards. Order, review, and interpret laboratory tests, diagnostic studies, and other clinical assessments. Provide preventative care, health screenings, and patient education. Monitor clients with chronic conditions such as hypertension, diabetes, asthma, and other common primary care conditions. Provide appropriate referrals to specialists and other healthcare professionals when needed. Coordinate care with behavioral health providers, specialists, case managers, and other members of the treatment team. Maintain accurate, timely, and complete electronic health records. Follow clinic policies, clinical protocols, HIPAA requirements, and applicable state and federal healthcare regulations. Participate in quality improvement, compliance, and care coordination activities. Communicate professionally and compassionately with clients, families, and members of the healthcare team. Qualifications Graduate of an accredited Family Nurse Practitioner program. Current and unrestricted Arizona Registered Nurse (RN) and Nurse Practitioner/APRN license, as applicable. Current national certification as a Family Nurse Practitioner (FNP) through an approved certifying organization. Current DEA registration if prescribing controlled substances is required for the position. Current BLS/CPR certification. Strong clinical assessment, diagnostic, and decision-making skills. Excellent communication and documentation skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Experience with electronic health records (EHR/EMR) preferred. Preferred Qualifications Previous experience in primary care, family medicine, community health, or integrated healthcare. Experience working with adults with behavioral health or substance use treatment needs is a plus. Experience working with Medicaid/AHCCCS populations is preferred. Knowledge of Arizona healthcare regulations and documentation requirements is beneficial.
