Wound Care Specialist Alveida Health Group Phoenix, AZ Job Details Full-time | Contract 1 hour ago Qualifications Wound infection prevention Certified Family Nurse Practitioner Wound care DEA License Full Job Description Alveida Health Group is seeking an experienced and compassionate Wound Care Specialist to join our healthcare team. The ideal candidate has strong clinical knowledge in wound assessment, treatment, prevention, and ongoing management and is committed to providing high-quality, client-centered care. Position Summary The Wound Care Specialist will evaluate and manage clients with acute, chronic, and complex wounds. This role includes performing wound assessments, developing treatment plans, providing appropriate wound care interventions, monitoring healing progress, educating clients and caregivers, and coordinating care with other healthcare professionals. The specialist will work collaboratively with primary care providers, nursing staff, behavioral health professionals, case managers, and outside specialists as needed. Responsibilities Perform comprehensive wound assessments and document wound characteristics, measurements, drainage, tissue type, and healing progress. Develop and implement individualized wound care treatment plans. Provide treatment for acute, chronic, surgical, pressure-related, diabetic, vascular, and other complex wounds within the provider's scope of practice. Perform wound cleansing, dressing changes, debridement, and other wound care procedures as permitted by licensure and training. Monitor clients for signs of infection, deterioration, or complications. Evaluate factors that may affect wound healing, including nutrition, circulation, mobility, diabetes, medication use, and underlying medical conditions. Recommend appropriate wound care products, dressings, and treatment modalities. Coordinate referrals to vascular, podiatry, infectious disease, surgery, primary care, or other specialty services when appropriate. Provide education to clients and caregivers regarding wound care, infection prevention, skin protection, nutrition, pressure relief, and follow-up care. Maintain accurate, timely, and complete clinical documentation. Collaborate with interdisciplinary team members regarding treatment goals and continuity of care. Follow infection-control standards, HIPAA requirements, clinic policies, and applicable state and federal regulations. Participate in quality assurance, compliance, and clinical improvement activities. Assist with developing or improving wound care protocols and procedures as needed. Qualifications Current and unrestricted professional license applicable to the position in the State of Arizona. Appropriate clinical background in nursing, advanced practice nursing, medicine, or another qualified healthcare discipline. Previous wound care experience required. Strong knowledge of wound assessment, treatment, infection prevention, and healing principles. Ability to recognize complications and determine when additional medical intervention or referral is needed. Excellent clinical documentation and communication skills. Ability to work independently and as part of an interdisciplinary team. Current BLS/CPR certification. Preferred Qualifications Wound care certification such as WCC, CWCN, CWS, CWON, or equivalent credential. Experience treating diabetic ulcers, pressure injuries, venous ulcers, arterial wounds, surgical wounds, and other complex wounds. Experience in primary care, outpatient care, home health, skilled nursing, rehabilitation, or integrated healthcare. Experience working with Medicaid/AHCCCS populations is preferred. Experience with electronic health records is preferred.
