Job Description

Astrana Care Hospitalists is looking for Physician Assistants and Nurse Practitioners to join our Post Acute Care program. We are looking for Physician Assistants and Nurse Practitioners who are willing to round at our local Skilled Nursing Facilities and see patients who have been recently discharged from our partnered hospitals. Our SNFs are located in the San Gabriel Valley, 15 minutes east of downtown Los Angeles, with easy access to the city, as well as other popular destinations such as Long Beach and Santa Monica. Astrana Care Hospitalists is partnered closely with Astrana Health and Allied Pacific IPA to provide high quality acute care to underserved populations in the San Gabriel Valley, with a strong focus on diversity, equality, and inclusion. The goal of the hospitalist program, as a whole, is to provide compassionate inpatient care to patients at contracted hospitals and SNFs year round, and to coordinate with primary care physicians, specialists, and all members of the healthcare team, to ensure smooth transitions of care.
Job Details:
We are currently looking for a Physician Assistant or Nurse Practitioner to cover Sunny Village Care Center, located in Alhambra, CA. SNFists will round on new and existing patients during the day, Monday to Friday Flexible schedule - round on patients at your convenience, as long as patients are seen within established time limits. On-call at home Malpractice is provided Competitive annual salary with potential bonuses and pay raises based on performance metrics such as length of stay and readmission rates
Requirements:
Board Certified as a Nurse Practitioner/Physician Assistant Certificate of completion from an accredited
Nurse Practitioner or Physician Assistant Program Current California State Medical License Current DEA Certificate Preferences:
Cantonese, Mandarin, and/or Spanish speaking are preferred, but not required
Pay:
From $150,000.00 per year
