Job Description Summary CCareMore Health is seeking a Field Nurse Practitioner to provide acute, post-acute, and transitional care services to medically complex and recently discharged patients in their homes throughout the San Fernando Valley and Antelope Valley. This field-based role combines clinical autonomy with interdisciplinary collaboration, allowing providers to deliver personalized, high-quality care that improves outcomes, reduces hospital readmissions, and helps patients recover safely where they are most comfortable-at home. Serving patients throughout the San Fernando Valley and Antelope Valley, you'll provide acute, post-acute, and transitional care while collaborating with an interdisciplinary team dedicated to delivering exceptional, patient-centered care. How You'll Make an Impact As a Field Nurse Practitioner, you'll provide high-quality acute, post-acute, and transitional care services directly in patients' homes. What We're Looking For Minimum Qualifications Master of Science in Nursing (MSN). Adult/Geriatric Nurse Practitioner certification. Active, unrestricted Registered Nurse (RN) license. Active, unrestricted Nurse Practitioner (NP) license in California. Minimum of one (1) year of Nurse Practitioner clinical experience. Strong assessment, diagnostic, and care management skills. Valid driver's license and reliable transportation. Ability to work independently while collaborating within a multidisciplinary team. Current TB test or willingness to obtain prior to employment. Compensation Range:
$119,313.00 to $178,968.00 The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
