Outpatient Primary Care / Community Health Position Overview We are seeking a dedicated Family Nurse Practitioner (FNP) to provide high-quality primary care services in a collaborative, patient-centered environment. The ideal candidate will be passionate about preventive care, chronic disease management, and improving health outcomes for diverse patient populations. Responsibilities Provide comprehensive preventive and therapeutic patient care. Evaluate, diagnose, and manage acute and chronic medical conditions. Develop treatment plans and coordinate referrals and follow-up care. Review laboratory results, imaging studies, and specialist recommendations. Educate patients on disease prevention, wellness, and treatment plans. Maintain accurate and timely clinical documentation using established standards. Collaborate with physicians and multidisciplinary healthcare teams. Participate in quality improvement initiatives, clinical meetings, and care team huddles. Support continuity of care through patient follow-up and care coordination. Follow approved clinical protocols and treatment guidelines. Qualifications Graduate of an accredited Nurse Practitioner program. Active California Nurse Practitioner license or eligibility for licensure. Current CPR/BLS certification. Primary Care or Family Practice experience preferred. Strong clinical, communication, and patient-care skills. Bilingual English/Spanish skills are a plus.
