Job Description

Southern California Medical Center is seeking an experienced Family Nurse Practitioner to join our healthcare team and provide high-quality urgent and primary care services at multiple clinic locations throughout Southern California. This is a traveling provider position supporting our locations in El Monte, Long Beach, and Pico Rivera. Weekend availability is required. Provider will travel between assigned sites as needed, and mileage is reimbursed for travel from one work site to another. English and Spanish bilingual proficiency is required. Position Responsibilities The Nurse Practitioner will provide comprehensive primary and urgent care services in accordance with standardized protocols and applicable California laws and regulations.
Responsibilities include:
Obtain complete patient histories and perform physical examinations. Evaluate, diagnose, and treat patients presenting with acute and chronic medical conditions. Develop, implement, and document individualized patient treatment plans. Order and interpret appropriate diagnostic tests, laboratory studies, and other medical evaluations. Perform therapeutic procedures, including immunizations, injections, and infection management. Prescribe medications within the scope permitted by California law, clinic policies, and standardized procedures. Exercise appropriate clinical judgment and consult with the Medical Director or other providers when necessary. Provide patient education and counseling regarding disease prevention, health maintenance, and treatment plans, including STD prevention and sexual health. Prescreen cases requiring review by the Chief Medical Officer or Medical Director. Maintain accurate, timely, and complete medical records. Participate in staff meetings, planning meetings, and other meetings as required. Participate in the organization's ongoing quality assurance and performance improvement activities. Provide culturally competent care to a diverse patient population. Maintain professional standards and comply with all applicable clinic, state, and federal requirements. Job Requirements Successful completion of an accredited Nurse Practitioner program. Current, unrestricted California Nurse Practitioner license. Current Nurse Practitioner board certification. Minimum 2 years of Nurse Practitioner experience required. Bilingual English/Spanish required. Experience in urgent care, primary care, community health, or a Federally Qualified Health Center (FQHC) setting is preferred. Ability and willingness to work weekends as required. Ability to travel between clinic locations in El Monte, Long Beach, and Pico Rivera. Valid driver's license and reliable transportation. Strong clinical, communication, documentation, and interpersonal skills. Ability to work effectively in a fast-paced, team-oriented clinical environment. Travel & Mileage This position requires travel between Southern California Medical Center locations in El Monte, Long Beach, and Pico Rivera based on operational needs. Mileage incurred while traveling from one work site to another for work purposes is reimbursed in accordance with company policy.
