Outpatient Primary Care + Assisted Living Facilities About Our Practice We are a physician-led medical group consisting of 2 physicians and 6 Nurse Practitioners/Physician Assistants providing care across multiple settings, including outpatient clinics, hospitals, skilled nursing facilities, and assisted living facilities throughout the Los Angeles area. We are looking for a motivated and personable Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in a primarily geriatric Internal Medicine/Primary Care role. This is a full-time, Monday-Friday position combining scheduled outpatient clinic days at our Inglewood office with visits to patients at local assisted living facilities. The position offers excellent autonomy, a supportive office staff, strong earning potential, physician availability, and a workflow designed to allow the provider to focus primarily on seeing patients and completing clinical documentation. Schedule & Work Setting This is a Monday-Friday position .
Your typical schedule will include:
Tuesday, Wednesday & Thursday:
Outpatient clinic in Inglewood Every other
Monday:
Outpatient clinic in Inglewood Remaining weekdays: Patient visits at assisted living facilities On assisted living days, you will typically visit one facility per day, occasionally two . If two facilities are scheduled, they will generally be located near each other to minimize unnecessary driving. The position therefore provides a nice combination of a traditional outpatient practice and the flexibility and autonomy of facility-based medicine. Typical patient volume is approximately 20-25+ patients per day , depending on the setting. A Provider-Focused Workflow One of the major advantages of this position is the support provided by our office staff. Our staff assists with many of the administrative and clinical tasks that can consume a provider's time, including: Rooming patients Taking vital signs Processing medication orders Prior authorizations Coordinating administrative requests Assisting with other routine office workflow This allows you to focus primarily on what you were trained to do: evaluate and manage your patients and complete high-quality clinical documentation. We also recognize that paperwork takes time. Providers receive an $500 per month paperwork stipend for time spent completing forms such as disability paperwork, IHSS forms, and other patient-related documents. Patient Population Our practice primarily serves an adult and geriatric population . We do not see patients under 18 years old , and this position does not involve women's health/OB-GYN care . Our patient population includes many older adults with chronic medical conditions who benefit from continuity of care and long-term relationships with their providers.
Responsibilities Your responsibilities will include:
Evaluate and manage adult and geriatric primary care patients Diagnose and manage acute and chronic medical conditions Perform routine follow-up and preventive care visits Evaluate patients at assisted living facilities Order medications, laboratory tests, imaging, and other diagnostic studies Review laboratory and diagnostic results Communicate with patients, families, caregivers, and facility staff Coordinate care with specialists and attending physicians Complete accurate, high-quality, and timely clinical documentation Complete necessary patient forms and paperwork EMR & Documentation Our outpatient practice uses Practice Fusion for electronic medical records. Prior Practice Fusion experience is helpful but not required. We will provide training. Our goal is to make the provider workflow as efficient as possible so that your primary responsibilities are seeing patients and completing your notes , rather than spending your day on administrative tasks.
Compensation Expected annual compensation:
$120,000•180,000+ Compensation is based on experience and patient volume/productivity . Providers who choose to see additional patients have the opportunity to earn more. In addition, we provide a $500/month paperwork stipend to compensate for time spent completing patient-related forms and administrative paperwork. This is a 1099 independent contractor position . Professional malpractice insurance is provided and paid for by the practice. Optional Weekend Hospital Coverage The primary position is Monday through Friday , and routine weekend work is not required . However, we also provide hospital-based care and would especially welcome a provider who is interested in helping with hospital coverage approximately one weekend per month . Weekend hospital coverage is optional and not a requirement for the position , but interest in hospital medicine and occasional weekend coverage would be a great bonus to our team. Autonomy With Physician Support This position provides significant autonomy while still giving you ready access to physician support. You will have independence in managing your patients and organizing your work, but an attending physician is available whenever questions or complex clinical situations arise. One of the attending physicians is physically present in the Inglewood office every Tuesday and is otherwise readily available by phone throughout the week for clinical questions and support. Qualifications Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in California Current DEA registration or ability to obtain one Experience in Internal Medicine, Primary Care, Family Medicine, or Geriatric Medicine is strongly preferred Comfortable managing multiple chronic medical conditions Strong clinical judgment and documentation skills Comfortable with EMRs and technology Reliable, organized, and responsive Strong communication and interpersonal skills Able to work effectively with physicians, office staff, patients, families, caregivers, and assisted living staff Who We Are Looking For We are looking for someone who is motivated, personable, friendly, dependable, and self-directed . The ideal candidate takes ownership of their patients, communicates well, works efficiently, and develops positive relationships with both patients and staff. It is particularly important that you work well with our office staff , as we have built a team-based workflow in which staff members handle many of the administrative tasks so that providers can focus on patient care. We are looking for someone who wants to become a long-term member of our practice and appreciates the combination of autonomy, physician support, schedule consistency, and a primarily geriatric patient population.
