Job Description

Job Summary Our client is seeking a dedicated Family Practice Nurse Practitioner to leverage modern technology in providing telehealth services. The primary responsibilities include conducting virtual visits, evaluating health risks, and managing patient care coordination. Key Responsibilities Review daily caseloads and prep technology for patient services. Use technology to manage schedules and coordinate with in-home clinical team members. Build connections with a diverse range of patients, gather health history, and prepare for virtual services. Conduct health risk assessments, medication reviews, and cognitive screenings. Collaborate with on-site team members to evaluate vitals and execute patient-tailored tasks. Manage preventive care, handle referrals, oversee medication management, and document clinical findings. Qualifications Board Certified as a Family Nurse Practitioner. Active Family Nurse Practitioner License in California. California DEA license. Credentialed Medicaid provider for the state of California (Medi-Cal). 1.5+ years of clinical experience in family practice, primary care, or internal medicine. Several years of telehealth experience preferred. Bilingual (Spanish-speaking) capabilities are highly valued. Hours Per Week 40.0 Benefits Paid Sick Leave (Medix provides paid sick leave according to state and local sick leave ordinances).
