Job Description

Location:
Montebello, CA Overview Provide comprehensive primary care to adult patients in ambulatory and community-based settings. Focus on preventive care, chronic disease management, acute care evaluation, and long-term wellness using evidence-based, patient-centered approaches. Responsibilities Evaluate, diagnose, and manage acute and chronic medical conditions in adults. Provide preventive care, health screenings, routine assessments, and immunizations. Initiate and manage medications; monitor treatment effectiveness and adjust plans as needed. Develop individualized care plans and deliver patient education on disease management and healthy behaviors. Coordinate care and consult with physicians and interdisciplinary team members. Maintain accurate, timely clinical documentation and adhere to clinic protocols and quality standards. Compensation & benefits
Salary:
$130,159-$195,235 annually (based on experience and qualifications) Medical, dental, and vision insurance Retirement plan Paid time off CME and professional development support Additional employee benefits Qualifications Master's or Doctoral degree from an accredited Nurse Practitioner program Current, unrestricted California Nurse Practitioner license National Board Certification as an Adult Nurse Practitioner or equivalent adult-focused primary care certification Current DEA registration or eligibility to obtain one
Preferred:
experience in ambulatory or community-based primary care Strong clinical assessment, diagnostic, communication, and patient education skills Commitment to evidence-based, patient-centered care
