Important to have proficiency in Cantonese and Mandarin speaking This position focuses on comprehensive patient assessment, diagnosis, treatment planning, and management of chronic diseases to support patient well-being in outpatient clinics. Offering both independent practice under supervision and a team-oriented environment, this role provides meaningful opportunities for clinical growth and professional development. Responsibilities Conduct thorough patient assessments and clinical diagnostics Develop and implement treatment plans tailored to patient needs Manage chronic diseases and provide ongoing patient education Oversee medication management and adjustments Deliver preventive care and wellness services Maintain accurate and timely documentation in Electronic Health Records (EHR) Required Qualifications Physician Assistant License (if applicable) Nurse Practitioner License (if applicable) Preferred Qualifications 1+ years of experience in family medicine Strong skills in patient assessment and clinical diagnostics Proficiency with Electronic Health Records (EHR) systems Experience managing chronic diseases Proficiency in Cantonese and Mandarin speaking
