We are looking for a motivated, self-directed clinician who can confidently manage a schedule across several nursing facilities, take ownership of their caseload, and consistently exceed performance goals. The position also provides genuine scheduling flexibility. Clinicians are expected to be present during each facility's core collaboration window of 10:00 a.m. to 2:00 p.m, but still have the autonomy to organize the rest of their day. You may start earlier or work later depending on your preference and scheduling needs. The Opportunity Many residents in skilled nursing and senior living communities cannot safely travel to an outpatient psychiatric office. Others have received behavioral healthcare primarily through telehealth despite needing in-person assessments and collaboration. We bring high-quality psychiatric care directly to the patients. Our clinicians do more than complete visits. They collaborate with facility teams, they audit and review for regulatory compliance, they share knowledge though in-services, and help build a better model of post-acute psychiatry. The result is elevated nursing teams, stronger care planning, fewer avoidable escalations, and improved quality of life for patients and the facility staff. What You'll Do Conduct in-person psychiatric evaluations and follow-up visits Diagnose and treat depression, anxiety, psychosis, mood disorders, dementia-related behaviors, and other psychiatric conditions Manage psychiatric medications and develop individualized treatment plans Treat acute and complex cases while providing preventive and long-term psychiatric care Monitor treatment response and adjust care plans when clinically appropriate Collaborate with nursing teams, attending physicians, primary care providers, families, and facility leadership Participate in care planning, discharge planning, psychiatric meetings, and interdisciplinary clinical discussions Communicate clearly with facility leadership regarding patients evaluated and recommended follow-up Educate patients, families, and facility staff Complete timely, clinically meaningful documentation that supports quality care and regulatory compliance Required Qualifications Active California license as a Nurse Practitioner or Physician Assistant DEA registration or the ability to obtain one At least two years of psychiatric or behavioral health experience Strong clinical judgment, communication, organization, and time-management skills Ability to manage an independent daily caseload and complete documentation on time Preferred Qualifications Psychiatric Mental Health Nurse Practitioner certification Physician Assistant CAQ in Psychiatry Experience in geriatrics, skilled nursing, post-acute care, or senior living Experience treating dementia-related behaviors and medically complex patients Who Will Thrive Here?
