Join a full-time mobile Nurse Practitioner team delivering relationship-centered care across San Gabriel Valley and East Los Angeles. You'll provide high-quality primary care through home visits, community outreach, and virtual appointments within a patient-focused, value-based organization that emphasizes prevention, proactive interventions, and coordinated care to improve long-term health outcomes. This role offers substantial clinical autonomy while collaborating with an interdisciplinary team committed to quality over volume—focused on preventing illness, managing chronic conditions, and supporting patients in their own environments. Position highlights
Schedule:
Full-time, Monday-Friday, 8:00 AM-5:00 PM Base salary: $130,159-$195,238 $10,000 sign-on bonus Mobile practice: home visits, community-based care, and telehealth Mileage reimbursement Quarterly incentive plan: up to 10% of quarterly earnings 160 hours PTO annually Comprehensive medical, dental, and vision benefits 401(k) with company match CME support: 5 days paid CME leave plus $2,000 reimbursement Autonomous practice environment with strong interdisciplinary support Value-based care model centered on preventive care, proactive interventions, and coordinated services Qualifications Active, unrestricted California Nurse Practitioner license Active DEA registration Current BLS certification Strong clinical assessment, diagnostic, and decision-making skills Able to practice independently and collaborate effectively with a care team Commitment to patient-centered, preventive, value-based care Excellent communication and relationship-building abilities Comfortable delivering care in home, community, and virtual settings
