$158,000

  • 185,000 annually (based on experience)

Monday-Friday Qualifications:
Required:
  • Active and unrestricted Nurse Practitioner (NP) license in the State of CA.
  • Master's Degree or Doctorate from an accredited Nurse Practitioner program.
  • Board Certification as a Family Nurse Practitioner.
  • Active and unrestricted DEA license.
  • Current BLS certification.
  • Minimum of two (2) years of Primary Care, Family Medicine, or Internal Medicine clinical experience.
  • Experience managing patients across the lifespan in an outpatient primary care setting.
  • Strong clinical assessment, diagnostic, treatment planning, and patient education skills.
  • Experience utilizing Epic Electronic Medical Records (EMR).
Preferred:
  • Previous experience working in a Federally Qualified Health Center (FQHC), Community Health Center, or other underserved healthcare setting.
  • Experience caring for diverse, multicultural, or multilingual patient populations.
  • Experience working with Medi-Cal patient populations.
  • Bilingual language skills.
  • Passion for providing equitable, community-focused healthcare.
Job Summary:
  • Provide comprehensive primary care services including patient assessments, preventive care, diagnosis, treatment, chronic disease management, and health screenings.
  • Perform comprehensive physical examinations, obtain medical histories, interpret diagnostic findings, and develop individualized treatment plans.
  • Prescribe medications, order diagnostic testing, and monitor patient progress while collaborating with physicians and interdisciplinary healthcare professionals.
  • Educate patients on disease prevention, medication management, lifestyle modifications, and overall wellness.
  • Coordinate referrals, specialty consultations, laboratory testing, diagnostic imaging, and follow-up care to ensure continuity of treatment.
  • Maintain accurate, timely, and complete documentation within the electronic medical record.
  • Collaborate with behavioral health providers, care teams, and community resources to deliver coordinated, whole-person care.
  • Provide care to approximately 16-18 patients per day while delivering compassionate, patient-centered services.
  • Travel between two nearby clinic locations as needed to support patient care and operational needs.
  • Participate in team meetings, cross-coverage for fellow providers, and ongoing quality improvement initiatives.
