This role offers the opportunity to deliver patient-centered care while supporting preventive health, chronic disease management, and long-term wellness. Responsibilities Provide comprehensive primary care services to adult patients. Evaluate, diagnose, and manage acute and chronic medical conditions. Perform preventive care, health screenings, and routine assessments. Manage medications and monitor treatment effectiveness. Develop individualized care plans and provide patient education. Support long-term disease management and promote healthy behaviors. Collaborate with physicians and other members of the healthcare team. Maintain accurate and timely clinical documentation. Compensation & Benefits $130,159 - 195,235 annually, based on experience and qualifications. Current, unrestricted California Nurse Practitioner license. National Board Certification as an Adult Nurse Practitioner or appropriate adult-focused primary care certification. Current DEA registration or eligibility to obtain one. Experience providing primary care in an ambulatory or community-based setting preferred. Strong clinical assessment, diagnostic, communication, and patient education skills. Commitment to evidence-based, patient-centered care.
