Job Description

Location:
West Los Angeles, California Role summary Deliver comprehensive primary care to adults in ambulatory and community-based settings, focusing on preventive care, chronic disease management, medication management, patient education, and coordinated team-based care. Key responsibilities Provide evaluation, diagnosis, and management of acute and chronic adult medical conditions Perform preventive care, routine assessments, and health screenings Prescribe, adjust, and monitor medications; assess treatment effectiveness Develop individualized care plans and provide patient-centered education and behavior-change support Coordinate care with physicians and multidisciplinary team members; refer as needed Maintain accurate, timely clinical documentation and follow regulatory/quality standards Compensation & benefits
Salary:
$130,159-$195,235 annually (based on experience/qualifications) Medical, dental, and vision insurance Retirement plan Paid time off CME and professional development support Additional employee benefits Minimum qualifications Master's or Doctoral degree from an accredited Nurse Practitioner program Current, unrestricted California Nurse Practitioner license National Board Certification as an Adult Nurse Practitioner or equivalent adult-focused primary care certification Current DEA registration or eligibility to obtain one Preferred Experience in ambulatory or community-based primary care Required skills Strong clinical assessment and diagnostic abilities Effective communication and patient education skills Commitment to evidence-based, patient-centered care
