This flexible position offers the opportunity to collaborate with healthcare professionals, utilize your extensive clinical skills, and make a meaningful difference in patients' lives. You will be responsible for conducting comprehensive assessments, managing, and supporting various specialties including primary care, Hormones, women and Men's health, and specialty clinics. Your expertise will help ensure high-quality, patient-centered care while maintaining compliance with healthcare regulations and documentation standards. Responsibilities Perform thorough patient assessments including vital signs, physical examinations, and medical history reviews using EMR/EHR systems. Prescribe medications and treatment plans in accordance with state laws and collaborative practice agreements, ensuring adherence to DEA regulations. Administer injections, and perform procedures such as suturing, and sterile processing. Manage acute pain cases, conduct triage assessments for urgent needs, and provide follow-up care via telehealth or in-person visits. Collaborate effectively with healthcare professionals across disciplines to coordinate patient care plans, discharge planning, case management, and utilization review. Document all patient interactions accurately within EMR. Requirements Current Li as a Nurse Practitioner or Physician Assistant with valid state certification.. Demonstrated ability to perform procedures such as venipuncture, pellets and Hormones , sterile techniques, Strong knowledge of Women's Health, medical terminology, pharmacology for medication administration and drug management. Excellent client communication skills with experience working with children, seniors in assisted living or nursing home settings, individuals with disabilities or developmental disabilities. Ability to work independently while collaborating within a multidisciplinary team; flexibility to adapt to varying clinical schedules on a PRN basis.
