2996661988-2 Apply for this job now Job Description Hartford Healthcare is seeking a Primary Care Nurse Practitioner or Physician Assistant to join our Healthcare & Medical Services team. In this role, you will provide comprehensive primary care, including health assessments, diagnosis, treatment, and patient education across a diverse patient population. You will collaborate closely with physicians and an interdisciplinary team to deliver high-quality, evidence-based care. Responsibilities Provide comprehensive primary care, including preventive, acute, and chronic disease management for adult patients Conduct detailed histories, physical exams, and develop evidence-based treatment plans Order, interpret, and act on diagnostic tests and imaging Prescribe medications and manage medication adjustments per current guidelines Coordinate care with physicians, nurses, specialists, and community resources Document all patient encounters accurately in the electronic health record (EHR) Educate patients and families on disease prevention, treatment plans, and self-management Participate in quality improvement, population health, and care management initiatives Utilize clinical decision support and telehealth tools as appropriate Maintain compliance with all regulatory, safety, and infection control standards Required Skills Primary care/Family or Internal Medicine clinical practice Chronic disease management (e.g., diabetes, hypertension, COPD, CHF) Physical examination and clinical assessment Diagnostic reasoning and clinical decision-making Ordering and interpreting labs and imaging Medication management and pharmacology Electronic Health Record (EHR) documentation (e.g., Epic or similar) Patient education and motivational interviewing Care coordination and population health management Telehealth/virtual visit proficiency
