2996669405-2 Apply for this job now Job Description Hartford Healthcare seeks a compassionate Hospice Admission Nurse to coordinate and complete timely, accurate hospice admissions. You will assess patients in homes and facilities, explain hospice services, obtain consents, and develop initial care plans. Working closely with an interdisciplinary team, you ensure smooth transitions, excellent symptom management, and education for patients and families. Responsibilities Conduct comprehensive nursing assessments for hospice eligibility in homes and facilities Educate patients and families on hospice philosophy, services, and care processes Obtain required consents and complete accurate admission documentation Develop initial, individualized plans of care in collaboration with the hospice team Coordinate with physicians, social workers, and other disciplines for smooth transitions Initiate symptom management and communicate urgent needs to providers Use EMR systems to document assessments, care plans, and communication Participate in case conferences and quality improvement initiatives Ensure compliance with Medicare, Medicaid, and regulatory Requirements Provide compassionate support and guidance to patients and caregivers Required Skills Comprehensive nursing assessment Hospice and palliative care principles Care planning and coordination Patient and family education Electronic Medical Records (EMR) documentation Symptom and pain management Knowledge of Medicare/Medicaid hospice regulations IV and medication administration Interdisciplinary team collaboration Time management and prioritization
