As part of one of the largest non-profit, fully-integrated healthcare systems in Connecticut, the facility utilizes state-of-the-art clinical technology including EPIC, promoting improved patient outcomes and streamlined continuity of care.

Primary Care Job Details:
Join an established family medicine practice caring for patients from birth through adulthood in a 100% outpatient setting Open to Family Nurse Practitioners (FNP) and Physician Assistants (PA) Full-time position with a four-day work week option available Weekdays only schedule with no call required Serve a diverse, multicultural patient population including many Spanish-speaking families Perform routine physical examinations and formulate medical, educational, and psychosocial treatment plans Develop plans for further evaluation of health needs, including referrals to specialists and other health team members Provide appropriate treatment for problems within scope of practice and refer outside cases to physician as needed Work collaboratively with practice physicians and care team members Unrestricted State of Connecticut Medical License required (or eligible) Minimum of 1+ years of experience in a primary care or similar setting required
